About This Role

The HUD-VASH Social Worker provides direct care and intensive case management services to Veterans experiencing homelessness and housing instability. This role involves implementing psychosocial treatment modalities, assisting Veterans in procuring necessary housing documents and resources, and supporting housing stability and community integration.

Responsibilities

  • Provide intensive case management services and implement psychosocial treatment modalities to patients and families experiencing psychiatric, medical, and socioeconomic problems.
  • Assist Veterans in procuring necessary documents for HUD Section 8 program applications, housing, and identified resources for community integration.
  • Provide case management services to support Veterans with housing stability and community integration.
  • Use evidence-based treatment modalities effective with the homeless population, including motivational interviewing, crisis intervention, trauma-informed care, harm reduction, and holistic whole health approaches.
  • Complete comprehensive bio-psychosocial assessments of Veteran households.
  • Establish and maintain effective professional working relationships with internal and external partners.
  • Document all work in the Electronic Health Record (EHR) within specified time frames.
  • Participate in weekly team meetings, community meetings, family conferences, and other endeavors to support housing stability.
  • Work with vulnerable Veterans with underlying chronic medical or mental health conditions.
  • Provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients.

Requirements

  • Master's degree in social work from a CSWE-accredited school
  • State licensure or certification to independently practice social work at the master's degree level (for GS-11 and above, or within 3 years for GS-9)
  • Proficiency in spoken and written English
  • Ability to work with Veterans and families from diverse backgrounds utilizing counseling skills
  • Ability to assess psychosocial functioning and needs and implement treatment plans
  • Ability to implement treatment modalities in working with individuals, families, and groups
  • Ability to establish and maintain effective working relationships and communicate with clients, staff, and community agencies
  • Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures (including acute, chronic, and traumatic illnesses/injuries, medications, and medical terminology) (for GS-9)
  • Knowledge of community resources, making appropriate referrals, and coordinating services (for GS-11)
  • Skill in independently conducting psychosocial assessments and treatment interventions (for GS-11)
  • Skill in independently implementing different treatment modalities (for GS-11)

Qualifications

  • Master's degree in social work from a CSWE-accredited school
  • Minimum of one year of post-MSW experience equivalent to the GS-9 grade level in health care or social work-related settings.

Nice to Have

  • Experience in a community-based mental health setting
  • Experience working with chronically homeless individuals
