RN - Hospice

About This Role

This role involves coordinating care, assessing patient and family needs, and implementing interdisciplinary care plans for hospice patients. The RN ensures all services align with physician's orders and regulatory standards.

Responsibilities

• Provide coordination of care
• Ensure continuous assessment of each patient's and family's needs
• Implement the interdisciplinary care plan
• Perform services in accordance with the physician’s orders and the established plan of care (POC)
• Ensure delivery of care is compliant with federal and state regulations

Requirements

• Licensed in the state of practice
• At least one year experience as a licensed professional
• Basic demonstrated technology skills, including operation of a mobile device
• Valid state driver license
• Maintain automobile liability insurance as required by law
• Maintain dependable transportation in good working condition
• Able to safely drive an automobile in all types of weather conditions
• CPR certification for the healthcare provider

Qualifications

• Graduate of an approved school of clinical education
• At least one year of experience as a licensed professional

Nice to Have

• Medicare home health or hospice experience
