Licensed Master Social Worker (LMSW) - Home Health - PRN

About This Role

Administer social services to home health patients by providing intermittent professional services and educating patients, families, and healthcare team members. Perform duties according to physician orders and established plans of care.

Responsibilities

• Administer social services to patients requiring intermittent professional services
• Teach patients, family, and other members of the health care team
• Perform services in accordance with physician’s orders and the established plan of care (POC)

Requirements

• Masters degree from a school of social work accredited by the Council on Social Work Education
• Certified or licensed in the state of practice
• At least one year of social work experience in a health care setting
• Basic demonstrated technology skills, including operation of a mobile device
• Valid state driver license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive an automobile in all types of weather conditions
• CPR certification

Qualifications

• Masters degree from a school of social work accredited by the Council on Social Work Education
• At least one year of social work experience in a health care setting

Nice to Have

• Medicare home health or hospice experience
