Licensed Master Social Worker

About This Role

Administer social services to patients requiring intermittent professional services, teaching patients, families, and healthcare team members. Perform duties in accordance with physician's orders and the established plan of care.

Responsibilities

• Administer social services to patients requiring intermittent professional services
• Teach the patient, family, and other members of the health care team
• Perform services in accordance with the physician’s orders and the established plan of care (POC)

Requirements

• Masters degree from a school of social work accredited by the council on social work education
• Certified or licensed in the state where they currently practice (Florida: No state licensure is required, certification only. Must provide masters degree transcript)
• At least one year of social work experience in a health care setting
• Basic demonstrated technology skills, including operation of a mobile device
• Valid state driver license
• Maintain automobile liability insurance as required by law
• Maintain dependable transportation in good working condition
• Able to safely drive an automobile in all types of weather conditions
• Possess CPR certification

Qualifications

• Masters degree from a school of social work accredited by the Council on Social Work Education
• At least one year of social work experience in a health care setting

Nice to Have

• Medicare home health or hospice experience
