Registered Nurse RN Home Health

About This Role

This Registered Nurse (RN) Home Health role provides direct patient care, coordinates with other healthcare disciplines, and ensures accurate clinical documentation to deliver high-quality home-based care.

Responsibilities

• Perform visits and complete coordination of client care
• Act as point of contact for all disciplines involved with providing care to patients
• Oversee the frequency of visits for the episode of care
• Consult with the physician and office regarding patient care details
• Perform accurate OASIS collection
• Ensure the medication profile remains current
• Ensure lab values have been reported to the physician timely
• Attend weekly case conference
• Attend monthly case manager meetings

Requirements

• Currently licensed as a Registered Nurse (RN) in the state of employment
• Demonstrate knowledge and skill in current nursing practice
• Possess a valid state driver’s license
• Automobile liability insurance

Qualifications

• A minimum of one year of clinical experience is preferred
