Registered Nurse (RN) Home Health

About This Role

This Registered Nurse (RN) position provides compassionate, personalized home healthcare to patients in the Walnut Creek area. The role involves administering treatments, monitoring conditions, creating care plans, and maintaining accurate records while collaborating with a supportive team.

Responsibilities

• Provide nursing care within the scope of practice, following the patient’s individualized care plan, in coordination with the Interdisciplinary Team
• Administer treatments and monitor patient conditions
• Report signs/symptoms signaling a change in patient condition to Physician, Case Manager, and/or Director of Nursing
• Maintain accurate clinical records and submit documentation according to company policy using a company-provided device
• Supervise Licensed Vocational Nurses (LVNs) when assigned
• Comply with all company policies and procedures to ensure high standards of care

Requirements

• Current Registered Nurse (RN) License to practice in California
• 1 year of experience in Home Health or Hospice care
• Current CA Driver’s License
• Ability to travel within a 15-25 mile radius on a regular basis

Qualifications

• 1 year of experience in Home Health or Hospice care
