Speech Therapist Home Health

About This Role

Join our Home Health team as a Speech Therapist to provide exceptional care, assess patient needs, develop individualized treatment plans, and enhance patients' quality of life. You will collaborate with the Clinical Director and Clinical Staff within a supportive work environment.

Responsibilities

• Provide, coordinate, and oversee skilled speech therapy services as per the patient’s individualized plan of care, in collaboration with the Interdisciplinary Team
• Conduct initial and ongoing evaluations of patient status
• Develop and implement treatment plans, monitor patient progress, and report any changes in condition to the Physician, Case Manager, and/or Director of Nursing
• Deliver educational information to patients and their families regarding speech and language disorders and treatment plans
• Maintain accurate clinical records, submitting documentation in a timely manner per company policy
• Adhere to all company policies and procedures to ensure the highest standards of care

Requirements

• Current OT License to practice in California
• 1 year of experience in Home Health or Hospice care
• Current CA Driver’s License
• Ability to travel within a 10-15 mile radius on a regular basis

Qualifications

• 1 year experience in Home Health or Hospice care
