About This Role

This Registered Nurse (RN) Home Health position is responsible for providing comprehensive client care, serving as the primary contact for all care disciplines, and overseeing visit frequencies. This role ensures accurate patient information and collaborates with physicians to deliver high-quality home-based care.

Responsibilities

• Perform visits and complete coordination of client care
• Serve as the point of contact for all disciplines involved with providing care to patients
• Oversee the frequency of visits for the episode of care
• Consult as needed with the physician and office regarding patient care details
• Perform accurate OASIS collection
• Ensure the medication profile remains current
• Ensure lab values have been reported to the physician timely
• Attend weekly case conference
• Attend monthly case manager meetings

Requirements

• Currently licensed as a Registered Nurse (RN) in the state of employment
• Demonstrated knowledge and skill in current nursing practice
• Possess a valid state driver's license
• Possess automobile liability insurance

Qualifications

• A minimum of one year of clinical experience is preferred
