As a Registered Nurse Case Manager specializing in Hospice, you will work with patients and their families during the final stages of life, providing compassionate care in various settings. This role involves supervising other nursing staff and communicating with healthcare professionals.

Responsibilities

• Work with patients, their families, and other healthcare professionals during the final stages of patients' lives
• Provide compassionate communication with patients and families
• Utilize keen observation skills to monitor patient conditions
• Alert doctors and others about changes in patients' conditions
• Supervise licensed vocational/practical nurses and nursing aides

Requirements

• Licensed as a Registered Nurse in the state of employment
• Valid state driver’s license
• Automobile liability insurance
• Dependable transportation

Qualifications

• Graduate of an approved school of nursing
• Minimum one year of clinical experience is strongly preferred

Nice to Have

• Minimum one year of clinical experience
• Previous home health or hospice experience
