About This Role

This RN Hospice case manager role involves providing compassionate care to patients in their homes, utilizing strong assessment and care planning skills. The role requires collaboration with the care team and effective patient/family education.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness
• Record pain/symptom management changes/outcomes as appropriate
• Teach patients, families, and caregivers appropriate patient care techniques
• Regularly communicate with clinical manager and care team
• Perform on-call responsibilities and on-call services to patients/families as assigned

Requirements

• One year of nursing experience
• Current RN license specific to the state(s) assigned to work
• Current CPR certification
• Valid driver's license
• Reliable transportation
• Liability insurance
