Registered Nurse Case Manager-Hospice

About This Role

As an Amedisys RN case manager, you will make a difference by helping patients heal at home. You will perform patient assessments, collaborate with the care team to develop and implement care plans, and provide on-call services to patients and families.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so
• Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness and according to care center policies
• Record pain/symptom management changes/outcomes as appropriate
• Teach patients, families, and caregivers appropriate patient care techniques
• Regularly communicate with clinical manager and care team
• Perform on-call responsibilities and on-call services to patients/families as assigned

Requirements

• One year of nursing experience
• Current RN license, specific to the state(s) you are assigned to work
• Current CPR certification
• Valid driver's license
• Reliable transportation
• Liability insurance
