RN Case Manager Hospice

About This Role

As an Amedisys RN Case Manager, you will make a difference every day by providing patient assessments, collaborating with care teams, and developing individualized care plans for hospice patients in their homes.

Responsibilities

• Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
• Make referrals to other hospice care disciplines as needed or document rationale for not doing so.
• Develop, prepare, and maintain individualized patient care progress records with accuracy and timeliness.
• Record pain/symptom management changes/outcomes as appropriate.
• Teach patients, families, and caregivers appropriate patient care techniques.
• Regularly communicate with clinical manager and care team.
• Perform on-call responsibilities and on-call services to patients/families as assigned.

Requirements

• One year of nursing experience
• Current RN license, specific to the state(s) you are assigned to work
• Current CPR certification
• Valid driver's license
• Reliable transportation
• Liability insurance

Qualifications

• One year of nursing experience.
