RN Case Manager Hospice

About This Role

As a Hospice Registered Nurse Case Manager, you will provide compassionate end-of-life care, coordinate services, and guide patients and families through their hospice journey. You will ensure patients receive The Best Care Possible, grounded in clinical excellence and connection.

Responsibilities

• Respond to clinical referral information with care and expertise, addressing inquiries and requests.
• Partner closely with the Clinical Director to assess client eligibility and suitability for hospice services.
• Coordinate and provide direct care as an RN Case Manager, ensuring patient comfort and quality of life.
• Apply nursing skills to deliver top-notch care rooted in proven principles and techniques to ease pain and bring solace.
• Empower patients and families through education, encouraging their active participation in creating personalized care plans.
• Participate in shared call duties to ensure continuous support when it matters most.

Requirements

• Current unencumbered Registered Nurse license
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting
• Computer proficiency, including typing and various programs, and background in EMR
• Current CPR certification if required by state
• Current driver's license, vehicle insurance, and access to dependable transportation

Qualifications

• Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
