As a Hospice Registered Nurse Case Manager, you will provide compassionate end-of-life care, coordinate services, and guide patients and families through their hospice journey. You will ensure patients receive The Best Care Possible, grounded in clinical excellence and connection.
Responsibilities
- Respond to clinical referral information with care and expertise, addressing inquiries and requests.
- Partner closely with the Clinical Director to assess client eligibility and suitability for hospice services.
- Coordinate and provide direct care as an RN Case Manager, ensuring patient comfort and quality of life.
- Apply nursing skills to deliver top-notch care rooted in proven principles and techniques to ease pain and bring solace.
- Empower patients and families through education, encouraging their active participation in creating personalized care plans.
- Participate in shared call duties to ensure continuous support when it matters most.
Requirements
- Current unencumbered Registered Nurse license
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting
- Computer proficiency, including typing and various programs, and background in EMR
- Current CPR certification if required by state
- Current driver's license, vehicle insurance, and access to dependable transportation
Qualifications
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting.
