About This Role

The Registered Nurse (RN) Home Health provides in-home patient care, coordinating with all disciplines involved in the patient's treatment plan. This role involves managing visit frequencies, accurate OASIS collection, and ensuring timely communication of patient details and lab values to physicians.

Responsibilities

• Perform visits and complete coordination of client care
• Act as the point of contact for all disciplines involved with providing care to patients
• Oversee the frequency of visits for each patient episode
• Consult as needed with physicians and the office regarding patient care details
• Perform accurate OASIS collection
• Ensure the medication profile remains current
• Ensure lab values have been reported to the physician timely
• Attend weekly case conference
• Attend monthly case manager meetings

Requirements

• Currently licensed as a Registered Nurse (RN) in the state of employment
• Demonstrated knowledge and skill in current nursing practice
• Possess a valid state driver's license
• Possess automobile liability insurance

Qualifications

• A minimum of one year of clinical experience is preferred
