Deliver compassionate, high-quality nursing care to patients in their homes, working weekends for Deaconess Home Health. The role involves managing a patient caseload, supervising other caregivers, and coordinating with multidisciplinary teams to ensure optimal patient outcomes.
Responsibilities
- Deliver compassionate, high-quality nursing care tailored to each patient’s needs
- Develop and adjust individualized care plans to ensure optimal patient outcomes
- Collaborate with physicians, therapists, and other healthcare professionals
- Supervise own caseload, nurturing the growth and development of LPNs, LVNs, and Home Health Aides
- Provide education and support to patients and their families
- Maintain thorough documentation to meet regulatory and operational standards
- Utilize the latest technology and best practices to enhance patient care
Requirements
- Active RN license in the state served
- At least 1 year of nursing experience
- Strong critical thinking, clinical expertise, case management, and a passion for patient care
- CPR/BLS certification
- Valid driver’s license
Qualifications
- Graduate of an accredited nursing program
- At least 1 year of nursing experience (Home Health preferred)
