RN Case Coordinator

About This Role

The RN Case Coordinator is responsible for assessing patient needs to determine the most appropriate level of care and coordinating services across the care continuum at Hartford Hospital. This role involves discharge planning, case management, and advocating for patients' smooth transition to the next level of care.

Responsibilities

• Assess patients for appropriate level of care
• Coordinate care and services across the care continuum
• Identify risk factors
• Facilitate involvement of support services
• Perform utilization management functions
• Develop and implement transition planning activities
• Utilize and document aspects of care in case management specific computer applications
• Collaborate with other health care team members on process and quality improvement initiatives

Requirements

• CT RN license
• ASN
• Two years acute-care, and diversified clinical nursing experience, including medical/surgical nursing
• Excellent communication, negotiation and conflict resolution skills

Qualifications

• ASN required, BSN preferred
• Two years acute-care, and diversified clinical nursing experience, including medical/surgical nursing

Nice to Have

• BSN
• Case management, home care, and/or managed care experience
• Knowledge of computer applications and reimbursement systems
• Certification in case management
