Implement and monitor patient care plans, serve as a primary coordinator for multidisciplinary care, and assess discharge planning needs for patients. This role is crucial in providing individualized care and maximizing staff performance within a comprehensive healthcare network.
Responsibilities
- Implement and monitor patient care plans
- Monitor, record, and communicate patient condition as appropriate
- Serve as a primary coordinator of all disciplines for well-coordinated patient care
- Note and carry out physician and nursing orders
- Assess and coordinate patient's discharge planning needs with members of the healthcare team
- Demonstrate knowledge and skills necessary to assess patients and to identify physical, psychosocial, cultural, spiritual and age-related needs in order to provide individualized care
- Delegate aspects of care to other nursing personnel consistent with their level of education and expertise
- Participate in daily interdisciplinary patient rounds
- Develop, implement, and revise individualized plans of care
- Participate in activities for professional growth and development outside of patient care
Requirements
- Current CT RN license
- Current BLS, ACLS, PALS
Qualifications
- Graduate of accredited nursing school with BSN preferred
Nice to Have
- PACU and or ICU/ ED experience
