About This Role

The Registered Nurse (RN) Cardiology Care Coordinator independently manages and coordinates care for patients with advanced cardiovascular disease, ensuring continuity of care and supporting clinical decision-making.

Responsibilities

  • Manage and coordinate care for patients undergoing evaluation and treatment for advanced cardiovascular disease, procedures, and therapies
  • Deliver patient education
  • Ensure continuity of care
  • Support clinical decision-making throughout the treatment process
  • Manage patient referrals
  • Monitor patient progress
  • Review test results
  • Support providers in developing treatment plans
  • Facilitate communication between patients, families, and the multidisciplinary care team
  • Participate in quality improvement initiatives to optimize outcomes and enhance patient experience

Requirements

  • English Language Proficiency
  • Graduate of a professional nursing school approved by ACEN or CCNE, or equivalent foreign nursing education
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. State, Territory, Commonwealth, or District of Columbia

Qualifications

  • Graduate of a school of professional nursing approved by ACEN or CCNE, or a Master's degree in nursing (MSN) or a related field with a BSN, or a Doctoral degree in Nursing or a related field with a BSN

Nice to Have

  • Minimum of 2 years experience in cardiac arrhythmia management
  • Experience coordinating care, including triaging alerts and managing consults
  • Working knowledge of heart failure management, including guideline-directed therapy, symptom monitoring, and patient education for HF self-care
  • Familiarity with telehealth workflows and remote monitoring for arrhythmia and HF patients
  • Competence in EHR documentation, consult management, and order entry
  • Skills in providing patient education and support related to cardiovascular health and treatment plans
  • Ability to manage patient calls, secure messaging, and alerts promptly and professionally
  • Experience working in interdisciplinary teams, including cardiology, Healthcare Technology Management, and nursing leadership
  • Flexibility to provide cross-coverage for HF clinic during absences and support structural heart or stress lab workflows as needed
