About This Role

Implement and evaluate patient care plans, performing assessments, diagnoses, and interventions for a designated patient group within an ambulatory medicine setting. Collaborate with healthcare teams to ensure high-quality patient outcomes.

Responsibilities

  • Conduct patient interviews, explain policies/procedures, and review patient charts
  • Assess gastrointestinal, cardiovascular, respiratory, renal, neurological health status, mobility, sensory deficits, prostheses use, skin condition, and pain management
  • Communicate and document patient's physiologic health status and plan of care
  • Assess patient’s psychosocial health status, coping mechanisms, knowledge level, and cultural/ethnic requirements
  • Identify patient outcome, develop criteria for measurement, and establish individualized patient goals
  • Identify actual/potential patient problems and patient’s need for teaching based on assessments
  • Implement the plan of care, maintaining constant vigilance over patient safety precautions
  • Administer prescribed treatments and use equipment necessary for patient care safely and efficiently
  • Provide emotional support to patient and significant others
  • Perform grade I-IV Decubitus Care, preventive skin care measures, and apply simple dressings
  • Administer medications correctly and safely, educating patients on drug and food interactions
  • Participate in patient and family education, providing explanations of conditions and discharge instructions
  • Collaborate with other care team members and physicians in planning and carrying out treatment regimens

Requirements

  • Graduate from an accredited School of Nursing
  • Current License to practice as a Registered Professional Nurse in New York State
  • Enrollment in an accredited BSN program within two (2) years of job entry date
  • Obtain a BSN Degree within five (5) years of job entry date

Qualifications

  • Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred.
