Referrals Registered Nurse

About This Role

This role involves responding to patient referrals for skilled home care services, evaluating appropriateness, and formulating comprehensive care plans in conjunction with the medical team, patient, and family. It requires collaboration to ensure timely and appropriate care transitions.

Responsibilities

• Respond to referrals in a timely manner and evaluate patients for appropriateness of skilled home care services
• Plan for the skilled home care needs of patients in a timely manner to ensure expeditious movement to the appropriate level of care
• Implement a comprehensive home care plan based on in-depth assessment of patient needs
• Coordinate activities with family, informal supports, and other community resources
• Maintain working knowledge of third-party reimbursement guidelines and regulation standards
• Function as a home care service resource person
• Perform job functions in a fiscally responsible manner consistent with the mission and vision
• Assist with orienting new nursing personnel to department policies, procedures, and practices

Requirements

• Graduate from an accredited School of Nursing
• Current License to practice as a Registered Professional Nurse in New York State

Qualifications

• Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred. Must be enrolled in an accredited BSN program within two (2) years and obtain a BSN Degree within five (5) years of job entry date.
• 1 year of critical care nursing experience strongly preferred

• 1 year of critical care nursing experience
