PRN/PT Social Worker MSW 1

About This Role

This role will provide essential social work services to patients and their families, addressing emotional and physical challenges that arise from various health conditions. Responsibilities include conducting psychosocial assessments, coordinating crisis intervention and discharge planning, and implementing treatment plans.

Responsibilities

• Provide patients and family members with education and advice to cope with emotional or physical problems
• Coordinate services including crisis intervention, bereavement, client advocacy, referrals, prevention, education, discharge planning and implementation
• Conduct and document detailed psycho-social assessments to interpret social, environmental, and medical needs of the patient
• Employ advanced practice methods in the development, implementation and evaluation of treatment plans
• Provide or supervise plan of care interventions including resource and referral assistance, patient advocacy, therapy, and counseling as appropriate
• Collaborate with the healthcare team, patient, and family in the development and implementation of plans
• Serve as a liaison with community resources to facilitate smooth transition and placement of patients
• Participate in performance improvement initiatives, including data collection, analysis, and continuing education programs
• Prepare and write professional reports, letters, and case summaries

Requirements

• Masters' in Social Work
• 1 year of experience in social work
• One of the following Texas state certifications: LCSW, LMSW, or LMSW-AP
