Pharmacist - Home Infusion

About This Role

This Pharmacist role involves patient outreach, education, and coordination of discharge planning to provide comprehensive care for home-based patients, primarily focusing on infusion services.

Responsibilities

• Conduct patient outreach and education regarding medication and infusion equipment
• Coordinate discharge planning with hospital and home health staff
• Perform care planning and clinical review for IV, enteral nutrition, and hospice patients
• Engage in multidisciplinary problem solving with clinical and logistical coordination
• Supervise and perform sterile drug compounding
• Conduct drug utilization review, both clinical and payer related
• Be available to work overtime or flexible hours as needed

Requirements

• Eligible for or hold a valid and current license as a Registered Pharmacist from the Indiana State Board of Pharmacy
• If eligible, must obtain Indiana licensure within 180 days
• Reliable transportation
• Proof of vehicle insurance
• Valid driver's license
• Excellent verbal and written communication skills
• Successfully pass a criminal background check
• Ability to lift, push, pull or handle at least 50 pounds

Qualifications

• Minimum of a B.S. degree in Pharmacy from an accredited school of Pharmacy. A Pharm.D. degree is preferred.
• One to two years home infusion or hospital pharmacist experience preferred.
