Patient Services Coordinator Home Health

About This Role

This role involves scheduling patients for home healthcare services, coordinating with various stakeholders to ensure seamless continuity of care, and managing communication tools for the field staff.

Responsibilities

• Schedule patients to branch field clinicians
• Communicate with field staff, patients, physicians, referral sources, caregivers, and other service providers to maintain proper care coordination and continuity of care
• Manage the on-call notebook and hospitalization logs to enhance communication among stakeholders

Requirements

• High school diploma or equivalent
• Licensed practical or vocational nurse in the state in which they currently practice, with at least one year of clinical experience in a healthcare setting OR at least one year of home health, hospice, or pediatric experience within the last 24 months
• Demonstrated understanding of staffing and scheduling requirements related to home care services
• Basic demonstrated technology skills, including operation of a mobile device
• Valid state driver license
• Automobile liability insurance as required by law
• Dependable transportation in good working condition
• Ability to safely drive an automobile in all types of weather conditions

Qualifications

• High school diploma or equivalent
• At least one year of clinical experience in a healthcare setting OR at least one year of home health, hospice, or pediatric experience within the last 24 months, with demonstrated understanding of staffing and scheduling in home care services.
