Patient Care Coordinator

About This Role

As a Patient Care Coordinator, you will ensure patients receive timely and well-coordinated care by scheduling procedures and managing details. You will collaborate with providers, patients, hospitals, and outside facilities to manage administrative processes.

Responsibilities

• Coordinate and schedule in-office and off-site procedures
• Communicate with patients, providers, and outside facilities to ensure everyone is informed
• Schedule with outside vendors, providers, and hospitals as needed
• Maintain provider schedules within the EMR system
• Prepare and send required documentation prior to procedure dates
• Obtain prior authorizations for applicable procedures
• Coordinate pre-op and post-op appointments
• Ensure patient accounts and scheduling details are accurate

Requirements

• Strong ability to work collaboratively with leadership, staff, and providers
• Excellent verbal and written communication skills
• Exceptional organizational and multitasking abilities
• Proficiency in Microsoft Office Suite
• High school diploma or equivalent
• Minimum of 2 years of experience in a medical office or clinic setting
• Availability of 30-40 hours per week
• Primary location: Oshkosh
