Outpatient Pharmacist

About This Role

This role involves providing pharmaceutical care and drug information to patients and medical staff at York Hospital. The Outpatient Pharmacist will compound preparations, advise on medication use, monitor drug therapy, and ensure proper controls of controlled substances.

Responsibilities

• Compound pharmaceutical preparations and/or label and package medications.
• Assist with and/or supervise IV preparation and delivery.
• Advise patients and medical/nursing staff on medication use including dosage, side effects, and interactions.
• Ensure proper controls and records of narcotics and controlled substances.
• Monitor drug therapy to identify, prevent or resolve problems.
• Provide clinical pharmacy services.
• Administer immunizations and recommend immunizations to patients.

Requirements

• Bachelors Degree in Pharmacy
• Less than 1 year relevant work experience
• Licensed Registered Pharmacist
• Basic Life Support certification
• Completion of degree program from accredited school of pharmacy

Qualifications

• Bachelor's Degree Pharmacy
• Less than 1 year relevant experience.
