Outpatient Pharmacist

About This Role

This role provides pharmaceuticals and drug information to patients and medical staff at York Hospital, ensuring proper medication use and safe drug therapy practices.

Responsibilities

• Compound pharmaceutical preparations and/or label and package medications and pharmaceuticals
• Assist with and/or supervise IV preparation and/or delivery
• Advise patients and medical/nursing staff on use of various medications including dosage, side effects, interactions
• Ensure proper controls and records of narcotics and other controlled substances
• Monitor drug therapy to identify, prevent or resolve actual or potential drug therapy problems
• Administer immunizations and review patient profiles for immunization qualification
• Assist with and/or supervise personal care/long term care medication delivery
• Assist in preparing required reports by compiling information

Requirements

• Completion of degree program from accredited school of pharmacy
• Licensed Registered Pharmacist upon hire
• Basic Life Support upon hire

Qualifications

• Bachelors Degree Pharmacy
• Less than 1 year relevant experience
