Nurse Practitioner (NP) - Hospice

About This Role

Provide medical care to hospice patients, determining eligibility and managing pain and symptoms. Collaborate as a member of the interdisciplinary team to ensure comprehensive patient care.

Responsibilities

• Provide medical care to hospice patients
• Determine hospice eligibility
• Manage pain and symptom management for hospice patients
• Participate in the care of the hospice patient as a member of the interdisciplinary team

Requirements

• Demonstrated knowledge of regulatory requirements, including Medicare conditions of participation
• Basic demonstrated technology skills, including operation of a mobile device
• Maintain automobile liability insurance as required by law
• Maintain dependable transportation in good working condition
• Able to safely drive an automobile in all types of weather conditions

Qualifications

• Graduate of an approved school, licensed as a nurse practitioner (NP)
• At least five years nursing experience with a minimum of one year in hospice preferred; at least three years of clinical registered NP experience preferred.
