Nurse Practitioner, Advanced Practice Provider - Bilingual

About This Role

As a Nurse Practitioner at Oak Street Health, you will deliver comprehensive, value-based primary care to Medicare-eligible adults. Your role involves providing longitudinal care, conducting office visits, and collaborating with a multidisciplinary team to ensure patients remain healthy and out of the hospital.

Responsibilities

• Provide exceptional primary care to adult patients.
• Conduct office visits for routine and acute healthcare issues.
• Administer Annual Wellness Visits and Health Risk Assessments, focusing on accurate documentation.
• Coordinate care with other providers, specialists, and testing facilities.
• Lead population health initiatives in coordination with the Care Team.
• Assist the care team with phone triage and outreach.
• Educate patients on their health conditions, care plans, and treatments.
• Participate in Oak Street Health promotional activities.
• Conduct home visits as needed.

Requirements

• US Work Authorization

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

Nice to Have

• Experience in primary care (internal medicine, geriatrics, or family medicine)
• Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments (HRAs)
• Passion for teamwork and cross-functional collaboration
• Desire to be part of an innovative population health model
• Bilingual proficiency in applicable areas
