As a Nurse Practitioner at Oak Street Health, you will deliver comprehensive, value-based primary care to Medicare-eligible adults. Your role involves providing longitudinal care, conducting office visits, and collaborating with a multidisciplinary team to ensure patients remain healthy and out of the hospital.
Responsibilities
- Provide exceptional primary care to adult patients.
- Conduct office visits for routine and acute healthcare issues.
- Administer Annual Wellness Visits and Health Risk Assessments, focusing on accurate documentation.
- Coordinate care with other providers, specialists, and testing facilities.
- Lead population health initiatives in coordination with the Care Team.
- Assist the care team with phone triage and outreach.
- Educate patients on their health conditions, care plans, and treatments.
- Participate in Oak Street Health promotional activities.
- Conduct home visits as needed.
Requirements
- US Work Authorization
Qualifications
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
Nice to Have
- Experience in primary care (internal medicine, geriatrics, or family medicine)
- Understanding of HCC documentation, ICD-10 Coding, and Health Risk Assessments (HRAs)
- Passion for teamwork and cross-functional collaboration
- Desire to be part of an innovative population health model
- Bilingual proficiency in applicable areas
