Nurse Practitioner, Advanced Practice Provider

About This Role

Provide effective and equitable value-based primary care to adults on Medicare, focusing on keeping them happy, healthy, and out of the hospital. This role involves independent patient care and collaboration with a multidisciplinary team to build meaningful patient relationships.

Responsibilities

• Provision of exceptional primary care
• Conduct office visits for routine and acute issues
• Administer annual wellness visits and health risk assessments, focusing on thoughtful, accurate, and specific documentation
• Care coordination with other providers, specialists, testing facilities, and agencies
• Lead population health efforts in coordination with the Care Team
• Assist the care team with phone triage and outreach
• Educate patients on their health conditions, care plans, and treatments
• Participate in Oak Street Health promotional activities
• Conduct home visits as needed

Requirements

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
• National certification in Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner
• Active, non-probationary state Nurse Practitioner license
• Active DEA license
• US Work Authorization
• Bilingual proficiency in Spanish

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate

Nice to Have

• Experience in primary care: internal medicine, geriatrics, or family medicine
• Understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 (International Classification of Diseases-10) Coding, and Health Risk Assessments (HRAs)
• Passion for teamwork and cross-functional collaboration
• Desire to be part of an innovative model focused on empirically-guided population health
