About This Role

Provide in-home health evaluations to help people bridge gaps in care and improve health outcomes. Conduct physical evaluations, medication and medical history reviews, and diagnostic tests using an iPad and intuitive clinical workflow.

Responsibilities

  • Engage with people in the comfort of their homes to provide in-home health evaluations
  • Build trusted relationships to make people healthier
  • Connect people to the right care for their unique needs
  • Travel locally and within licensed states as needed
  • Conduct virtual visits as needed
  • Provide all Signify Health services including Diagnostic & Preventive Services products (e.g., spirometry, DEE, bone density screening)

Requirements

  • Active, unrestricted license(s) in coverage area(s)
  • Board certification
  • Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke, and other potential airborne allergens or irritants

Nice to Have
