Responsibilities

• Conduct in-home visits focusing on a person’s overall health and well-being
• Identify potential barriers to care during visits
• Perform medication and medical history review
• Conduct a physical evaluation in the health plan member’s home
• Utilize pre-populated, patient-specific forms for each evaluation

Requirements

• Board-certified Nurse Practitioner
• State Nurse Practitioner License
