Responsibilities
- Conduct in-home visits focusing on a person’s overall health and well-being
- Identify potential barriers to care during visits
- Perform medication and medical history review
- Conduct a physical evaluation in the health plan member’s home
- Utilize pre-populated, patient-specific forms for each evaluation
Requirements
- Board-certified Nurse Practitioner
- State Nurse Practitioner License
