Provide skilled nursing care to patients in their homes across Broward County, monitoring conditions, administering medications, and maintaining accurate documentation. This role focuses on compassionate, high-quality care within an employee-friendly organization.

Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with the Plan of Care
• Monitor patient condition and report changes to the supervising nurse or clinical team
• Administer medications and perform treatments within scope of practice
• Maintain accurate and timely clinical documentation
• Communicate effectively with patients, families, and interdisciplinary care team members

Requirements

• Active Florida LPN license
• Strong clinical, communication, and documentation skills
• Ability to work independently in the field
• Reliable transportation
• Valid driver’s license

Qualifications

• Prior home health experience preferred
