Licensed Vocational Nurse (LVN)

About This Role

The Licensed Vocational Nurse (LVN) will deliver high-quality, evidence-based patient care by assessing patient needs, administering treatments and medications, and collaborating with interdisciplinary teams. This role utilizes critical thinking and clinical knowledge to support improved patient outcomes and enhance healthcare delivery within the community.

Responsibilities

• Administer oral, injectable, intravenous, and topical medications.
• Evaluate complex patient conditions, monitor pain levels, and document treatment responses.
• Apply advanced knowledge of vital sign monitoring to identify and record abnormalities.
• Perform routine assessments to monitor patient health and ensure overall well-being.
• Manage patient access to healthcare and connect individuals with community resources.
• Conduct medication reviews to identify potential side effects and drug interactions.
• Develop and administer mental health assessments to screen for depression.
• Accurately document patient medical history using electronic health records.
• Interact with patients, families and other healthcare professionals.

Requirements

• Knowledge of medical terminology and clinical documentation standards
• Experience with phlebotomy, vaccine administration, and point-of-care testing
• Excellent communication and organizational skills
• Experience working in a clinical setting with diverse cultural dynamics
• Proven ability to manage sensitive information ethically and responsibly
• Strong problem-solving, critical thinking, and decision-making skills
• Proficiency with electronic medical record (EMR) systems

Qualifications

• High school diploma or GED and graduate of a State approved Practical or Vocational nursing program.

Nice to Have

• Epic EMR experience preferred
