This role is for a Licensed Practical Nurse (LPN) to provide direct patient care, administer medications, and assist in monitoring patient health at Stratford Manor Rehabilitation & Care Center.
Responsibilities
- Assist CNAs in performing ADL and routine care
- Conduct resident/patient treatments
- Administer medication in accordance with physician orders and Plan of Care
- Submit pharmacy orders
- Accurately document and record all information
- Give injections of medication as prescribed
- Observe resident health to communicate current condition to RNs, Supervisors, and Physicians
- Take vital signs, including blood pressure, temperature and weight
- Perform basic care, including wound cleaning and bandaging
- Manage IVs, starting IV drips
- Monitor fluid/food intake and output
- Observe and report resident accidents, incidents and the presence of skin breakdowns
- Assist as directed in the admissions process
Requirements
- Licensed Practical Nurse (LPN)
Qualifications
- New Nurse graduates and LPNs with experience are welcome.
