Responsibilities
- Provide skilled nursing care in clients' homes including medication administration, wound care, and vital signs monitoring
- Support clients' emotional and physical well-being through genuine connection
- Maintain clear documentation and communicate updates to the care team
- Educate families and clients on health conditions and self-care practices
- Assist in hands-on Care Professional training and ensure care compliance
Requirements
- Current LPN license in South Dakota
- Compassionate, patient-focused approach to care
- Strong communication and interpersonal skills
- Ability to work independently and collaboratively
- Reliable transportation and willingness to visit client homes
