This role involves performing patient screenings, assisting with patient care planning and implementation, and acting as a nursing resource for clinical staff in accordance with Pennsylvania State Board of Nursing provisions. The nurse will provide care at an intermediate level, utilizing sound nursing judgment and clinical expertise.
Responsibilities
- Obtain patient history and gather information
- Perform specialized procedures, examinations, and/or tests as ordered and aligned with the Ambulatory Clinical Scope Document (e.g., urinary catheterization, IV insertion with approved certification)
- Ensure all patient care areas are adequately stocked with unexpired supplies and medications
- Sterilize instruments, equipment, and supplies as required, assessing sterility and ensuring proper set-up
- Relay information and facilitate communication between the provider and the patient
- In collaboration with Physician/APP, reinforce health education, counseling, and advice to patients and families on topics such as healthy lifestyle changes
- Evaluate patient’s/caller’s health status based upon presenting symptoms and follow defined protocols to determine urgency for care
- Track, review, and monitor the follow-up of diagnostic test results to ensure physician/APP verification and patient notification
- Assist in the management of patient utilization of the health care system and provide patient education relative to health care needs
- Complete necessary paperwork, prior authorizations, and/or scheduling of appointments for patients ordered diagnostic and therapeutic services
- Maintain patient records to ensure accurate documentation of medical treatment
- Adhere to Health Insurance Portability and Accountability Act (HIPAA) of 1996
- Document and process prescription requests and refills per Physician/APP order or policy
- For HBO Wound Care: Potentially take a 40-hour introductory course for hyperbaric medicine and monitor hyperbaric dives
- For HBO Wound Care: Perform transcutaneous oxygen monitoring testing, blood glucose monitoring, and hyperbaric treatments
- For HBO Wound Care: Educate patient and/or patient’s family on benefits, risks, contraindications, and safety issues of hyperbaric oxygen therapy
- For HBO Wound Care: Evaluate patient outcome and meet with manager and hyperbaric physician to discuss and adjust treatment
Requirements
- Diploma or Associate's Degree from an accredited LPN program
- 1 year clinical nursing experience
- Licensed Practical Nurse (PA or Multi-State) or Registered Nurse (PA or Multi-State)
- Basic Life Support certification
- Excellent interpersonal/communications skills
- Models teamwork
- Self-motivated and dependable
- Able to work independently
- Advanced skills and good communication
Qualifications
- Diploma Program Certificate of completion (degree or diploma) from an accredited LPN program or Associate's Degree Certificate of completion (degree or diploma) from an accredited LPN program
- 1 year clinical nursing experience
