Join a skilled nursing team at Highland Chateau Health + Rehabilitation as a Licensed Practical Nurse (LPN). This role involves providing high-quality direct nursing care to residents, administering medications, and coordinating with interdisciplinary teams to ensure care plans are followed.

Responsibilities

• Provide direct nursing care to residents in accordance with their care plans
• Administer medications and treatments as prescribed by physicians
• Monitor residents’ conditions and report any changes to the RN or physician
• Document all nursing interventions and resident responses accurately and promptly
• Assist with assessments, admissions, discharges, and transfers of residents
• Collaborate with CNAs and other staff to ensure the highest quality of care
• Maintain a clean, safe, and supportive environment for residents
• Provide education to residents and families regarding health conditions and care needs
• Uphold all state, federal, and facility regulations and standards of care
• Participate in ongoing training and quality improvement initiatives

Requirements

• Current and valid LPN license in the state of Minnesota
• Graduation from an accredited practical nursing program
• CPR/BLS certification
• Strong clinical, communication, and critical thinking skills
• Compassionate, reliable, and team-oriented attitude

Qualifications

• Graduation from an accredited practical nursing program
