Licensed Practical Nurse

About This Role

This role will work as part of a care team to deliver exceptional patient care and improve clinical efficiency within a physician practice setting. The Licensed Practical Nurse will focus on enhancing the overall patient experience and coordinating care across the continuum.

Responsibilities

• Assist provider by collecting patient history and reason for visit, completing observation, physical exam, telephone triage, and analysis of diagnostic data
• Ensure all ordered laboratory and diagnostic tests are tracked, logged, and presented to the provider with all results
• Provide education to patients, ensuring materials are current and effective
• Assist patients with inquiries for prescription refills, appointment scheduling, and referral authorization
• Assist providers during procedures and examinations, and perform instrument and room clean-up
• Coordinate internal and external appointments for new and established patients based on medical needs, preferences, and practice schedule
• Intervene to prevent, minimize, or correct any actual or potential patient risks based upon observation of physiological, behavioral, and environmental factors
• Participate in telephone triage by obtaining clinically appropriate data and symptoms, advising and instructing patients according to established protocols or provider orders

Requirements

• Technical School Diploma LPN Program
• Ability to establish and maintain control of violent patients or dangerous situations
• Basic computer skills
• Critical thinking skills
• Ability to concentrate
• Knowledge and competence reflecting current nursing practice
• American Heart Association Basic Life Support certification in Pennsylvania
• Valid LPN license in Pennsylvania
