Licensed Practical (LPN) or Registered Nurse (RN)

About This Role

Provide daily clinical care to individuals, including medication administration and vital checks, while closely monitoring health status and responding to changes. This role ensures the safety and well-being of the people supported by acting as a proactive health advocate and building strong relationships.

Responsibilities

• Provide expert clinical care and consultation, including assessment, care planning, medication administration, and physician order execution
• Advocate for individuals' health by scheduling, coordinating, transporting, and communicating with healthcare providers
• Monitor health status, recognize changes in condition, and maintain thorough healthcare documentation and records
• Empower Med Aides, support staff, and individuals through health-related training and education (e.g., infection control, medication administration)
• Build and maintain effective therapeutic relationships with individuals and their support circles

Requirements

• State license in nursing
• Bachelor's or Associate's degree
• Ability to lift, push, and pull at least 50 pounds
• Valid Driver's license

Qualifications

• Bachelor's or Associate's degree in nursing
• 1 year of general nursing experience, or 3 years CNA/CMA experience can substitute if a new LPN; new graduates welcome
