Licensed Practical (LPN) or Registered Nurse (RN)

About This Role

Provide essential, daily clinical care including medication administration, tube feedings, and taking vitals, while closely monitoring health status and promptly responding to changes for individuals supported by Mosaic.

Responsibilities

• Provide expert clinical care and consultation by delivering precise nursing activities including assessment, care planning, medication administration, and follow-up on physician orders.
• Act as a proactive health advocate by ensuring individuals receive necessary routine and preventative healthcare through scheduling, coordinating, transporting, and communicating with healthcare providers.
• Monitor and manage health status by providing ongoing assessment, recognizing changes in condition, and maintaining thorough healthcare documentation.
• Empower through education by delivering health-related training on infection control and medication administration to Med Aides, support staff, and the people served.
• Build strong relationships with individuals and their support circles, including family and caregivers, to maintain effective therapeutic relationships.

Requirements

• State nursing license
• Ability to lift, push, and pull at least 50 pounds
• Valid Driver's license

Qualifications

• Bachelor's or Associate's degree in nursing
• 1 year of general nursing experience, or 3 years CNA/CMA experience if a new LPN, new graduates welcome
