Lead Hospice GIP RN

About This Role

This is a leadership opportunity for a Lead Hospice GIP Nurse to guide patients and families through critical moments and coordinate seamless transitions across various care settings.

Responsibilities

• Perform coordination of services to provide a seamless transition from acute and long-term care to Home Care
• Complete clinical review of data collected and accurately determine appropriateness and acceptance of referrals based on established criteria
• Coordinate services and disseminate information, and facilitate transfer of referral to other agency for declined referrals
• Serve as an information resource for all RHCD services
• Provide exceptional customer service by tone of voice, willingness to help, and using clinical knowledge
• Accurately and efficiently enter and retrieve data from various computer systems
• Work closely with care management team/discharge planners to identify potential referrals for RHCD services
• Build comprehensive knowledge base and continuously work toward remaining knowledgeable and current of Medicare/Insurance information and regulations
• Perform patient interviews and/or family conferences to inform of ordered services and discuss insurance coverage
• Provide leadership to and supervision for the RHCD Patient Transition Coordinators

Requirements

• 1 year Nursing experience
• Program Graduate, Nursing
• Associates Degree, Sociology/Social Work/Nursing
• Registered Nurse (RN) - Virginia Department of Health Professions (VDHP)
• CPR/BLS Certification

Qualifications

• Program Graduate, Nursing and Associates Degree, Sociology/Social Work/Nursing
• 1 year Nursing experience required
