This role involves conducting in-home health evaluations for health plan members as a Full Time Clinician (Nurse Practitioner or Physician Assistant). You will build trusted relationships, bridge gaps in care, and help improve health outcomes by providing comprehensive evaluations and education in people's homes.
Responsibilities
- Provide In-Home Health Evaluations to health plan members in their homes
- Conduct virtual visits as needed
- Perform medication and medical history reviews
- Complete a physical evaluation
- Administer additional diagnostic tests (e.g., diabetic eye exams, spirometry) if ordered by health plan
- Use an iPad and intuitive clinical workflow for evaluations
- Travel locally and within licensed states as needed
- Provide all Signify Health services including Diagnostic & Preventive Services products
Requirements
- Active, unrestricted license(s) in coverage area(s)
- Board certification
