Hospice Care Registered Nurse

About This Role

This role involves performing age-specific patient care using the nursing process, collaborating with healthcare team members, and utilizing leadership skills to coordinate and delegate patient care activities within a hospice setting.

Responsibilities

• Conduct pre-admission screening and initial nursing health assessment on new patients, assessing body systems (cardiac, vascular, neurological, and gastrointestinal, etc.)
• Develop nursing plan of care and goals based on patients needs
• Provide nursing care to patients including supervising diet, medication, activity level, safety in the home, and activities of daily living
• Evaluate the provision of nursing care by physical, occupational, speech, nutrition, and respiratory therapists, and contracting agency nurses
• Train and evaluate home health aides, homemakers, and personal care workers in the patient’s home
• Determine the need for, and recommend the rental or purchase of medical equipment and/or supplies for patients
• Provide on-going evaluation of equipment for defects, repairs or additional needs
• Recommend to the Hospice IDG (Interdisciplinary Group) when various therapies are warranted based on clinical assessment
• Report to and consult regarding patient status, and participate in on-going orientation and in-service training
• Participate in on-call coverage system on a rotating basis and serve as a liaison for maintaining the Hospice patients Plan of Care

Requirements

• Graduate from an accredited School of Nursing
• Current License to practice as a Registered Professional Nurse in New York State
• 2+ years of nursing experience

Qualifications

• Graduate from an accredited School of Nursing. Bachelor’s Degree in Nursing, preferred.
• 2+ years of nursing experience with a minimum of one year in acute care – med/surg strongly preferred.
