Provide comprehensive inpatient and outpatient care within the Heart Failure Management Program at Hartford Hospital, working closely with Heart Failure Cardiologists and a multidisciplinary team. This role helps patients with advanced heart failure using various resources.
Responsibilities
- Provide comprehensive inpatient and outpatient care in the heart failure management program
- Work closely with Heart Failure Cardiologists
- Participate in a multidisciplinary team including advanced practice providers, physicians, dieticians, nurses, and medical and administrative assistants
- Actively manage heart failure related volume overload and reduce hospital admissions through programs like Bridge Infusion, HF Clinic, PA Sensor, and VAD Program
Requirements
- Graduate of an accredited Physician Assistant / Nurse Practitioner Master’s program
- Licensed, or eligible to be licensed, as a Physician Assistant / Advanced Practice Registered Nurse (APRN) in the state of Connecticut
- Certification or eligible to be certified as a Physician Assistant or National Board Certification as an Advanced Practice Registered Nurse
Qualifications
- Graduate of an accredited Physician Assistant / Nurse Practitioner Master’s program
- Minimum 1-2 years of experience in cardiology, critical care, or advanced heart failure preferred. New grads with strong clinical backgrounds may be considered.
Nice to Have
- 1-2 years of experience in cardiology, critical care, or advanced HF
- LVAD experience
