About This Role

The Community Resource Specialist conducts resource assessments and collaborates with social work and medical staff to secure community resources for Medical Center patients and families. The role also provides orientation and training related to community resources to various stakeholders.

Responsibilities

  • Determine resources to optimally meet patients' and families' needs, utilizing knowledge of local and state resources to develop a multifaceted service plan.
  • Maintain a caseload with multiple patient contacts daily, including phone calls, paperwork, and face-to-face interactions.
  • Function as the primary healthcare contact for psychosocially complex patients when necessary.
  • Provide timely and appropriate documentation in patients' medical records (paper and electronic).
  • Advocate on patients' behalf, assisting with accessing entitlement programs and providing medical information to external agencies.
  • Conduct staff and community-based training on resources, providing updates on procedural requirements and changes from state and local agencies.
  • Document and advise supervisors and clinical staff on obstacles to obtaining services.
  • Develop liaisons with community and governmental agencies to facilitate negotiation of complex systems and broaden knowledge.

Requirements

  • High School diploma or GED
  • 1-3 years related work experience (2 years if no Bachelor's degree)
  • Experience with computer systems, including web-based applications and Microsoft Office (Outlook, Word, Excel, PowerPoint or Access)

Qualifications

  • High School diploma or GED required. Bachelor's degree preferred in Social Work or Human Services
  • 1-3 years related work experience required, or 2 years of related work experience without a related Bachelor's degree.

Nice to Have

  • Bachelor's degree in Social Work or Human Services
  • Licensed Social Worker
