Community Health Worker

About This Role

This role supports vulnerable community members by addressing material and health challenges through referrals to support services and in-home visits. The Community Health Worker will mentor others and promote well-being among members.

Responsibilities

• Serve as a mentor for Community Health Workers
• Provide oversight in care of members to improve health outcomes and promote well-being
• Act as a resource for other Community Health Workers and members
• Provide support and basic health education about maternal health, general healthcare, and community resources
• Conduct one-on-one health education, community outreach, regular in-home visits, and service coordination
• Provide various supports, services, entitlements and benefits for members
• Support members in developing health management care plans
• Empower members by promoting effective approaches, problem solving, and lifestyle changes
• Establish positive and supportive relationships and effectively work with diverse members
• Deliver coaching and mentorship on chronic health conditions and self-care
• Enter client data into software on a daily basis
• Advocate for members and their families, collaborating with the care team as needed

Requirements

• High School Diploma or equivalent
• 1-3 years of relevant experience
