The Community Care Coordinator Nurse delivers knowledge-based care to assigned clients, focusing on assessing, planning, implementing, and evaluating the effectiveness of the Veteran's care. This role supports the delivery of high-quality, timely care to Veterans receiving services in the community and coordinates care with an emphasis on patient education and satisfaction.
Responsibilities
- Delivers knowledge-based care to assigned clients while developing technical skills
- Assesses, plans, implements, and evaluates the effectiveness of the Veteran's care
- Serves to support delivery of high quality, timely care to the Veteran receiving services in the community
- Assumes responsibility for the coordination of care focused on patient education and customer satisfaction throughout the continuum of care
- Influences care outcomes by collaborating with members of the interdisciplinary team
Requirements
- English Language Proficiency
- Graduate of a professional nursing school accredited by ACEN or CCNE
- Current, full, active, and unrestricted registration as a graduate professional nurse
Qualifications
- Graduate of a school of professional nursing approved by an appropriate accrediting agency (ACEN or CCNE), or a Master's degree in nursing, or a Doctoral degree in Nursing.
