The Clinical Nurse II on the Acute Rehab Unit at UCI Health will collaborate with interdisciplinary teams to develop and implement comprehensive care plans for assigned patients. This role involves performing professional nursing duties, assessing and advocating for patients, evaluating outcomes, and serving as a role model and mentor to new nurses and students.
Responsibilities
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Perform professional nursing duties incorporating psychomotor and age-appropriate cognitive skills
- Assess, plan, intervene, and advocate for patients throughout the continuum of care
- Evaluate patient outcomes, plan of care effectiveness, and readiness for discharge
- Revise care plans or make referrals as necessary
- Act as a role model and mentor to students and new nurses
- Function as a Patient Care Coordinator (PCC) as requested
Requirements
- Minimum 2 years inpatient clinical nursing experience in adult acute care
- Minimum 1 year Rehabilitation experience
- Bachelor of Science in Nursing
- Ability to work variable shifts including evenings, nights, weekends, and holidays
- Ability to read, write and communicate effectively in English
- Ability to maintain flexibility and work well in a constantly changing environment
Qualifications
- Bachelor of Science in Nursing
- Minimum of 2 years recent inpatient clinical nursing experience in adult acute care and 1 year of Rehabilitation experience
Nice to Have
- Understanding of applicable regulatory requirements
- Bilingual skills to communicate effectively with patients and families
