About This Role

Provide Chronic Care Management (CCM) services to patients, focusing on managing chronic conditions and improving health outcomes. Work under the direct supervision of a physician, displaying responsible behavior and effective communication as part of the healthcare team.

Responsibilities

  • Greet and escort patients to exam rooms
  • Collect patient information and assessment data, including vital signs
  • Perform Wellness Visits
  • Assist physician with procedures, treatments, and examinations
  • Maintain principles of aseptic techniques and infection control
  • Ensure accurate and timely patient chart entries
  • Provide ongoing monitoring and follow-up of patients' progress and adherence to care plans
  • Educate and empower patients and caregivers to self-manage conditions
  • Confirm and clarify written orders prior to implementing delegated tasks
  • Perform delegated procedures such as injections and nebulizer treatments
  • Observe, record, and report patient's condition and reaction to drugs and treatments to physician
  • Follow policies for infection control in handling and disposing of infectious/hazardous waste materials
  • Appropriately obtain and manage specimens
  • Recognize emergency situations and implement emergency procedures
  • Provide appropriate patient-specific education and health information materials
  • Make, receive, and handle medically related phone calls as directed by physicians
  • Order, stock, and maintain exam room supplies
  • Assist receptionist in scheduling appointments as needed
  • Complete all logs as required by OSHA Regulations
  • Count and record all injectable medications daily
  • Inspect all sample and injectable medications weekly and dispose of expired drugs
  • Notify patients of test results and schedule follow-up appointments
  • Schedule CT scans, MRIs, EEGs, etc. according to physician's orders
  • Supply patients with written instructions for preparation for special procedures or surgeries
  • Conduct Remote Patient Monitoring (RPM)'s for physicians

Requirements

  • Nationally recognized certification as Medical Assistant OR two years of medical assistant experience
  • Ability to communicate effectively with others in the healthcare team
  • Understanding of medical terminology, anatomy and physiology, diagnostic procedures, pharmacology, and treatment assessments
  • Skills in applying and modifying principles, methods, and techniques of professional nursing to provide ongoing patient care
  • Ability to work as a team player
  • Ability to multi-task and prioritize multiple projects
  • Maintain strict confidentiality and professionalism
  • Compliance with HIPAA confidentiality standards

Nice to Have

  • Previous medical office experience
  • EMR experience
  • Personal experience in care management, preferably in cardiology or chronic disease management
