About This Role

Respond to inquiry calls, conduct initial intake assessments, and coordinate admissions processes for patients. This role manages patient flow from initial inquiry through admission or referral to appropriate care levels.

Responsibilities

  • Respond to inquiry calls and complete inquiry forms accurately
  • Complete initial intake assessments in a timely manner and document follow up calls on pending assessments
  • Input initial referral requests into EMR system to initiate insurance verification
  • Conduct phone assessments and complete Intake Screening and Assessment Part 1 on accepted patients
  • Upload pertinent referral documentation into document storage in EMR system
  • Refer patients not meeting facility criteria to appropriate community resources or outpatient programs
  • Communicate with patients, families, referral sources, and internal personnel in a professional, culturally sensitive manner
  • Deliver patient services respectfully, promoting dignity and patient satisfaction
  • Identify safety needs, possible signs of abuse/neglect, and report to Supervisor
  • Contact emergency services for patients in crisis, keeping them engaged until help arrives
  • Identify special mobility/equipment needs and need for interpreter services prior to admission
  • Assess and interpret age and disability needs to determine level of care and place patients at appropriate facilities
  • Communicate with providers regarding patient presentation for acceptance/denial decisions
  • Maintain real-time progress of referrals and update facilities, requesting assistance from Clinical Liaisons when necessary

Requirements

  • RN, LPN, Paramedic or EMT licensure
  • Understands admission criteria for varying levels of care, medications, and community resources
  • Interprets medical clearance and discerns acutely medical inappropriate values
  • Possess interpersonal skills to develop rapport with patients, family members, and referral sources
  • Able to assess and interpret data about patient's status to identify needs and appropriate referrals
  • Demonstrate knowledge of mental illness, chemical dependency, and family dynamics
  • Excellent communication skills related to age specific patient needs
  • Proficient assessment skills
  • Versatile, flexible and able to perform in stressful situations
  • Good time management and organizational skills
  • Able to work with mentally/physically disabled persons
  • Able to read, write and follow verbal and written instruction

Qualifications

  • Minimum of 2 years' experience in Mental Health/Chemical Dependency field preferred
